If your child has severe allergies, there are several things we can do to help at school. Your child may have their EPIPEN at school as well as over-the-counter medication they take to treat an allergic reaction. At the elementary age all medicine including EPIPENS are kept in the clinic. Their EPIPEN will also be sent on all Field Trips with their teacher. We also ask that you fill out a Severe Allergy Parent Questionnaire and a Severe Allergy Action Plan below and bring them to me. If your child's allergy is to a food, please have your physician complete the Nutrition Services Physician's Statement listed below. These forms are standard for every school in CSISD and will need to be filled out yearly. This will help us know the treatment plan your doctor has prescribed for maintenance and in case of emergency!
Your child's medical condition will also be communicated to their teacher so he/she will be aware and alert to any signs and symptoms that would be important to their care. I am always in the clinic or available by radio. Our campus is small enough that I can get anywhere very quickly should your child need assistance.
