If your child has diabetes, we can safely help manage it at school. We have had several children at Creek View with diabetes. Depending on the level of independence in their management they can come to the clinic for every check all the way to checking on their own in the classroom. It all depends on their ability to manage their diabetes and you and your doctor's consent. We have had children on multiple shots a day at school and students with insulin pumps. We can work with wherever your child is on his/her journey with diabetes! We also use a web-based program called Blue Loop that allows me to put in your child's pertinent numbers for the day (BG, Carbs, etc.) so that you can receive them in real time via text message or email. Click on the button below to view the site. We ask that you fill out a Diabetes Authorization for Administration of Management and Care. We also need the Diabetes Management and Treatment form filled out by your child's diabetes doctor. These forms are standard for every school in CSISD and will need to be filled out yearly. Please bring these forms to me so we can put a plan in place before the school year starts or before your child returns to school if they are diagnosed during the school year. This will help us know the treatment plan your doctor has prescribed for maintenance and in case of emergency! Your child's medical condition will also be communicated to their teacher so he/she will be aware and alert to any signs and symptoms that would be important to their care. I am always in the clinic or available by radio. Our campus is small enough that I can get anywhere very quickly should your child need assistance.
